You should try what Darek suggested, if that dosent work, all i can think of is reloading a save before you talked to smiley. I went back to town and the lady forgot her name still asks me about Smiley. Actually I kept my save on one file, but when I unlocked the elevator and went down, smiley was there and then he followed me back to the exit and I completed the quest.
